Adjuvant Therapy of Colorectal Cancer



Abstract
Chemotherapy, radiotherapy, and immunotherapy have been applied, in various doses and combinations, as adjuvant to surgical resection in the management of colon and rectal cancers. A prospective trial was undertaken to reevaluate the adjuvant
chemotherapy for the colorectal cancer, and adjuvant radiotherapy for the rectal cancer. Eligibility for trial of adjuvant chemotherapy was Dukes B and C cancer removed surgically with negative or equivocal margins of resection(182 patients). The
Groupings(5 groups) were control group(surgical treatment only. N=56), 5FU group(24 hour continuosly given twelve mg of 5 FU/kg body weight for five consecutive days, and repeated the course 6 times in the period of six months, N=56), 5FUFT
group(5FU IV
same as in 5FU group in 1st cycle and then Futrafur 1200 mg p.o. per day for one year, N=36), 5FUACNU group(5FU IV same as in 5FU group and ACNU 100 mg/§³ of body surface in 1st day cycle, and repeated the course 6 times in the period of six
months,
N=20) and MMCFTOK group(mitomycin C 6 mg IV/day two times in a week until total dose of 40 mg with OK-432 1.0 KE s.c. and Futrafur 1200 mg p.o. per day for one year, N=14). The adjuvant chemotherapy for Dukes B and C colorectal cancer such as
5FU,
5FU
and furtrafur. 5FU and ACNU, Futrafur and mytomycin C with OK-432 did not improve significantly the cumulative survival more than surgical resection only. The combination therapy with 5FU and other drug did not give more survival than 5FU only.
Eligibility for trial of adjuvant radiochemotherapy in rectal cancer was Dukes C cancer removed surgically with negative or equivocal margins of resection(30 patients). Groupings were control groups(surgical treatment only, N=15) and RT+5FU
group(postoperative radiotherapy 5,500 cGy by 4 weeks after operation, and 24 hour continuously given twelve mg IV of 5 FU/kg body weight for five consecutive days, and repeated the course 6 times in the period of six months, N=15). The four year
survival rate, 61.1% of the group of postoperative radiotherapy and chemotherapy with 5FU was more than that, 35.8%, of surgical resection only. But it was not stastically significant .
